Where your mouth meets

the rest of you

A Miami functional dentist treats the mouth as part of one connected system, using diagnostics like saliva analysis to find what is driving decay, bleeding gums, and lingering inflammation before repairing the damage it left behind. Dr. Niloufar Rezakhani, DMD, built her side of our practice around that sequence: understand the cause, then treat it.

Our team has worked this way for years across Miami Beach, North Miami, and Hollywood. What Dr. Rezakhani sharpens is the biological side of it, the chemistry and the bacteria of your mouth, and how both shift with the way you eat, breathe, and sleep.

The systemic thread we keep pulling

Gum tissue is living and vascular, which is why oral health keeps surfacing in research about the rest of the body. Bleeding gums are an open wound that bacteria and inflammatory byproducts cross every day. The acidity of your saliva decides whether enamel rebuilds overnight or keeps losing ground. The bacteria living on your tongue take part in chemistry your body uses well beyond your mouth.

We are dentists, so we treat what sits in our lane: the bacteria, the chemistry, and the tissue both affect. When your findings point somewhere outside it, we say so and work alongside your physician.

If what is bothering you is mechanical rather than biological, a jaw that aches, headaches, a bite that has never felt right, that is a different evaluation and it has its own page.

How does Guided Biofilm Therapy clean differently?

Guided Biofilm Therapy is a hygiene protocol that finds plaque before removing it. We apply a disclosing dye first, which stains biofilm a visible color so you can see exactly where it collects in your own mouth. Then we lift it away with a warm spray of water, air, and fine erythritol powder, reaching below the gum line where a toothbrush never gets. Ultrasonic instrumentation follows for hardened calculus, so scaling stays limited to the spots that genuinely need it.

Two things make the switch worth it for our patients. The visit is gentler because most of the work happens with powder and water instead of metal against enamel. It’s also teachable. Once you have watched the dye light up the same three spots twice, brushing instructions stop being abstract.

Published comparisons of erythritol air-polishing with ultrasonic instrumentation have found it as effective as conventional scaling and root planing, with less chair time and better patient-reported comfort.

What can saliva testing

tell us?

Saliva testing reads the environment your teeth and gums live in. Three markers guide what we recommend afterward.

pH

A mouth that sits acidic for long stretches loses minerals from enamel faster than it rebuilds them. Saliva that buffers well does the opposite, and knowing which one describes you changes how aggressive prevention needs to be.

Microbial markers

The species tied to decay behave differently from the ones tied to gum inflammation. Seeing which population dominates tells us whether to aim at the enamel or at the tissue around it.

Nitrites

Bacteria on the back of your tongue convert dietary nitrate into nitrite, which the body can then use to make nitric oxide. Studies have found this nitrate-reducing population depleted in people with periodontitis, and it is one reason we think twice before sending anyone home with a harsh antiseptic rinse.

A saliva panel is a risk-assessment tool. It is not a diagnosis of any medical condition, and we do not present it as one. What it does is aim prevention with some precision: two patients with nearly identical x-rays can need very different advice once we know what their saliva is doing.

PRP and regenerative healing

For surgical cases we use platelet-rich plasma drawn from your own blood, combined with laser therapy, to reduce inflammation and support healing and integration.

Know your real risk

Two patients with nearly identical x-rays can need completely different prevention. Testing tells us which one you are, so what we recommend is aimed rather than generic.

Calm inflammation at its source

Removing biofilm precisely, and knowing which bacteria we are up against, does more than a deeper scrape.

Keep more of your natural tooth

Catching the chemistry before the cavity means fewer drills, smaller restorations, and less lost enamel.

Advanced technology

for better care

We diagnose with equipment that shows considerably more than a standard exam can.

  • Saliva testing: Reads the pH, bacterial populations, and nitrite activity that decide how your mouth behaves between visits.
  • Guided Biofilm Therapy system: Disclosing dye plus air-polishing, so we find biofilm before we remove it.
  • Fotona laser: Handles non-invasive treatment with no downtime and no bleeding.
  • Intraoral scanner and cancer screening lights: Support early detection and accurate records without discomfort.

For bite, jaw, and airway measurement, including CBCT imaging, EMG muscle testing, jaw tracking, and T-Scan bite analysis, see our physiologic dentistry page.

Frequently asked questions

How is this different from physiologic dentistry?

Functional dentistry works on the biology: the bacteria in your mouth, the chemistry of your saliva, and the inflammation both create. Physiologic dentistry works on the mechanics: your bite, jaw joints, muscles, and airway. Many patients need both, and your evaluation tells us where to start.

What does functional dentistry cost?

Cost depends on what your evaluation finds, so we quote after the exam instead of before it. We take Visa, Mastercard, American Express, and Discover; work with most PPO insurance plans and Medicare, and offer third-party financing for plans you would rather spread out. Our team can check your coverage before you commit to anything.

Who is a good candidate for this approach?

Patients who keep getting cavities despite brushing well, whose gums bleed through every cleaning, or who have been told everything looks fine while something clearly isn’t. Seeing a Miami functional dentist also makes a reasonable starting point if you want a conservative second opinion before agreeing to extensive work.

How long does it take?

A first evaluation runs longer than a routine exam, since we are gathering more: a comprehensive exam, imaging, and any saliva testing we recommend. Guided Biofilm Therapy fits a normal hygiene appointment. We review testing results with you once they are back and go through what each finding changes about your plan.

Does Guided Biofilm Therapy hurt?

Most of the work happens with a warm spray of water, air, and erythritol powder, so there is far less scraping than in a conventional cleaning. Sensitive teeth and inflamed gums tend to tolerate it well. Tell us if anything feels uncomfortable, and we will adjust as we go.

Will saliva testing tell me whether I have a medical condition?

No. It reports on the chemistry and bacteria of your mouth, which is dental information. If a result or an exam finding suggests you should speak with your physician, we’ll say so openly and share what we found. We don’t diagnose or treat conditions outside the mouth.

What happens at a first visit?

You get a comprehensive oral exam, intraoral x-rays, and a conversation about your history, your habits, and what has not been working before anyone recommends treatment. We list what to bring and what to expect for your first visit, including how we handle insurance verification.
